Individuals with chronic kidney disease (CKD) may develop end stage renal disease (ESRD) requiring renal replacement therapy (RRT) by dialysis and/or transplantation, and are at higher risk of major cardiovascular events.
Evidence from the UK East Midlands suggests that many CKD patients are undetected, and that those needing secondary care are referred late and thus exposed to worse clinical outcomes. Separation between secondary and primary care introduces obstacles into the patient pathway which also contribute to poor outcomes. The NHS 5 year forward plan has a strong focus on better collaborative working to try and overcome these barriers.
